Concessionary Travel Scheme

Eligibility

You are eligible to apply if your sole or main residence is within Adur and is not a second home and qualify under one or more of the following :

Note : There is a minimum age of 5 if an application is to be made under one of the disability criteria.


Blind or partially sighted

You will qualify under this section if you are registered with Social Services or, if you are not registered, you meet the requirements for registration with Social services, as :

  • 'Blind' or 'severely sight impaired (blind)' means having a high degree of vision loss (i.e. seeing much less than is normal or perhaps nothing at all)

Blind

You would be able to register as 'Blind' if :

  • You cannot see (with glasses, if worn) the top letter of the eye test chart (used by doctors and opticians) at a distance of 3 metres or less.
  • You can read the top letter of an eye test chart at 3 metres, but not at 6 metres, and your field of vision is severely restricted.

If you qualify under this section, you will need to provide proof of eligibility 'Blind registration card' or 'a Certificate of Vision Impairment (CVI) signed by a Consultant Ophthalmologist’ or ‘in receipt of a guaranteed income payment for tariff levels 1-8 under the 2005 Armed Forces Compensation scheme’. Alternatively, if you have none of these, your consultant/specialist can certify your eligibility by completing a form that can be downloaded below or obtained by contacting us on (01273) 263399. You will need to complete the declaration of authority section before passing this to your consultant/specialist.

Partially sighted

You will qualify under this section if you are registered with Social Services or, if you are not registered, you meet the requirements for registration with Social services, as :

  • 'Partially sighted' or 'sight impaired (partially sighted)' is a less severe loss of vision. Partially sighted people can see more than someone who is blind, but less than a fully sighted person.

You would be able to register as 'Partially sighted' if :

  • You have a full field of vision, but can only read the top letter of the eye test chart at a distance of 6 metres or less (with glasses, if worn).
  • You can read the next three lines down at the same distance and your field of vision is either moderately or severely restricted.

If you qualify under this section, you will need to provide proof of eligibility 'Partially sighted registration' or 'a Certificate of Vision Impairment (CVI) signed by a Consultant Ophthalmologist’ or ‘in receipt of a guaranteed income payment for tariff levels 1-8 under the 2005 Armed Forces Compensation scheme’. Alternatively, if you have none of these, your consultant/specialist can certify your eligibility by completing a form that can be downloaded below or obtained by contacting us on (01273) 263399. You will need to complete the declaration of authority section before passing this to your consultant/specialist.

Having a long-term disability, which substantially impairs ability to walk

You will qualify under this section if :

  • You are unable to walk a single step or your only way to get about is to swing through crutches, or
  • (Whether with or without an aid) you cannot walk for distances over 64 metres without severe discomfort at the time or later as a result of walking the 64 metres, or
  • You cannot walk 100 metres within 5 minutes, or
  • You are unable to walk very far and as a direct result of the effort required, is likely to lead to a serious deterioration in your health, needing medical intervention for you to recover, or
  • The effort required to walk would constitute a danger to your life.

You will not be eligible if you have suffered from the disability / injury for less than 12 months.

If you qualify under this section, you will need to provide proof of eligibility 'Disability Living Allowance (higher rate mobility component) book / letter' or ‘in receipt of a guaranteed income payment for tariff levels 1-8 under the 2005 Armed Forces Compensation scheme’ or your consultant/specialist can certify your eligibility by completing a form that can be downloaded below or obtained by contacting us on (01273) 263399. You will need to complete the declaration of authority section before passing this to your consultant/specialist.

Profoundly or severely deaf

You will qualify under this section if :

  • You have a severe hearing loss of 70 - 95 dBHL, or
  • You have a profound hearing loss of 95+ dBHL

If you qualify under this section, you will need to provide proof of eligibility 'Disability registration with Social services' or ‘a report from an aural specialist or Audiologist’ or ‘in receipt of a guaranteed income payment for tariff levels 1-8 under the 2005 Armed Forces Compensation scheme’ or your consultant/specialist can certify your eligibility by completing a form that can be downloaded below or obtained by contacting us on (01273) 263399. You will need to complete the declaration of authority section before passing this to your consultant/specialist.

Without speech

You will qualify under this section if :

  • You are unable to make clear basic oral requests (e.g. to ask for a particular destination or fare), or
  • You are unable to ask specific questions to clarify instructions (e.g. Does this bus go to the High Street?)

You will not be eligible if you can communicate orally but your speech might be slow or difficult to understand because, for example, you might stammer.

If you qualify under this section, you will need to provide proof of eligibility 'Disability registration with Social services' or ‘a report from a speech specialist’ or ‘in receipt of a guaranteed income payment for tariff levels 1-8 under the 2005 Armed Forces Compensation scheme’ or your consultant/specialist can certify your eligibility by completing a form that can be downloaded below or obtained by contacting us on (01273) 263399. You will need to complete the declaration of authority section before passing this to your consultant/specialist.

Significant learning disability

You will qualify under this section if :

  • Your disability started before adulthood and has a lasting effect on your development.
    AND
    • You have a reduced ability to understand new or complex information, or
    • You have difficulty in learning new skills, or
    • You are unable to cope independently

If you qualify under this section, you will need to provide proof of eligibility 'Disability registration with Social Services' or ‘a report / assessment from a medical or social care Learning Disability specialist’ or your consultant/specialist can certify your eligibility by completing a form that can be downloaded below or obtained by contacting us on (01273) 263399. You will need to complete the declaration of authority section before passing this to your consultant/specialist.

Refused a driving licence on medical grounds

You will qualify under this section if :

  • You have had an epileptic attack whilst awake within the last year, or
  • You have a history of epileptic attacks whilst asleep and have had one whilst awake in the last 3 years, or
  • You have not had an epileptic attack whilst awake in the last 3 years, but would likely to be a danger to the public if driving, or
  • You have a severe mental disorder, or
  • You are liable to sudden attacks of giddiness or fainting (whether as a result of cardiac disorder or otherwise), or
  • You are unable to read a registration plate in good light at 20.5 metres (with lenses if worn), or
  • You have another disability which is likely to cause driving a vehicle by you to be a source of danger to the public.

You will not be eligible to apply if you persistently misuse drugs or alcohol and this has caused one of the above conditions.

If you qualify under this section, you will need to provide proof of eligibility 'a DVLA document confirming that you are ineligible for a driving licence on long term medical grounds (D206 or D235)’ or ‘a report from a medical specialist’ or ‘in receipt of a guaranteed income payment for tariff levels 1-8 under the 2005 Armed Forces Compensation scheme’. Alternatively, your consultant/specialist can certify your eligibility by completing a form that can be downloaded below or obtained by contacting us on (01273) 263399. You will need to complete the declaration of authority section before passing this to your consultant/specialist.

Without the use of both arms

You will qualify under this section if :

  • You have had both arms amputated or
  • You have a congenital absence of both arms or
  • You have a deformity of both arms or
  • You have one or both arms but are unable to use any to carry out day to day tasks (e.g. paying coins into a fare machine) or
  • You have muscular dystrophy, spinal cord injury, motor neurone disease or a condition of comparable severity.

If you qualify under this section, you will need to provide proof of eligibility 'Disability registration with Social Services' or 'War Pensioners Mobility Supplement' or ‘a report from a medical specialist’ or your consultant/specialist can certify your eligibility by completing a form that can be downloaded below or obtained by contacting us on (01273) 263399. You will need to complete the declaration of authority section before passing this to your consultant/specialist.
