How to apply
Step one :
Before you apply make sure you have read the following :

Step three :
- Complete your form.
- Enclose a copy of your evidence of eligibility (see 'evidence of eligibility' below). Please do not send original documents as these may get lost in the post. The copies you send in will be securely destroyed unless you specifically request them to be returned to you. If you bring in your evidence of eligibility it can be verified at the time and copies will not need to be produced.
- Return your completed application form to the Concessionary Travel Team at the Adur Civic Centre.
Please note :
- The information on the application form will be entered on a database and used to maintain the concessionary travel records.
- The information you supply will be cross-checked against Council Tax and Housing Benefit records to confirm eligibility and prevent fraud.

Evidence of eligibility :
- Sole or main residency : eg, council tax bill from the current financial year, a utility bill from the last three months, current television licence stating your name and address or a letter from the local authority confirming you are a resident at the address within that council area.
- AND age : eg, valid passport, driving licence, birth certificate, other government document that shows your date of birth.
- OR disability (depending on category) :
- Disability Living Allowance (higher rate mobility component),
- Higher rate Attendance Allowance,
- Social Services disability registration (blind, partially sighted, without arms, deaf, without speech, learning disability),
- Certificate of Vision Impairment (CVI),
- DVLA document,
- DSS benefit (war pensioners mobility supplement), in receipt of a guaranteed income payment for tariff levels 1-8 under the 2005 Armed Forces Compensation scheme,
- Specialist's report,
- or your consultant/specialist can certify your eligibility by completing a form as outlined in the appropriate disability or companion section.
Photographs :
If you are applying for a bus pass, you will also need to include a coloured passport sized photo :
- The photo must be 35mm wide by 45mm high
- You must have a light background behind you
- The photo must stand out clearly against the background
- The photo must be undamaged (ie no creases caused by paper clips)
- Other people or objects must not be visible in the photo
- The photo must be properly focused and clear
- The person must be facing forward and looking straight into the camera
- Expression must be neutral
- There must be no sunglasses, tinted glasses or hair obscuring your eyes
- The photo must show the applicant’s full head without any covering, unless the covering is worn for religious beliefs or medical reasons and subject to the point immediately below
- The photo must be taken with nothing covering the outline of the applicant’s nose, eyes and mouth
This photo can be attached to the application form or by sending Concessionary Travel an e-mail quoting your name and address.
Alternatively you can come into the Adur Civic Centre to have your photo taken digitally, free of charge.
